I understand that MIDLETON IT is not an authorized service dealer, and that repairs rendered by MIDLETON IT may void manufacturer warranties for this device. MIDLETON IT does not assume any liability or warranty in the event that the manufacturer warranties are voided.
I understand that MIDLETON IT offers no verbal or written warranty, either expressed or implied, regarding the success of this repair and I expressly waive all claims against MIDLETON IT for any damages to this device or data that is lost due to the repair rendered by MIDLETON IT.
I fully understand that I, the owner of this device, have backed up all information on the hard disk in which I want to preserve. I also understand that MIDLETON IT is not responsible for any lost data, and waive any legal action against this company.
understand that the data on this device is not the responsibility of MIDLETON IT, and may be lost during the repair process without my prior knowledge.
I understand that as a result of submitting my personal device for repair that it could be subject to loss, theft, damage, or data loss.
Our Technicians will not browse through your hard drive or memory cards looking at your data; however, they may inadvertently see data during the course of their work. Please remove any personal or private files you do not want others to see.
Customer agrees to resolve all disputes with MIDLETON IT through binding arbitration. By signing repair paperwork, you waive your right to a lawsuit.
Further, I agree to release, indemnify, and hold MIDLETON IT from liability for any claims or damages of any kind or description that may arise from any repair work performed on my electronic device, unless it is caused by severe negligence of MIDLETON IT or its agent.
SIGNING/LEAVING YOUR DEVICE WITH US FOR REPAIR CONFIRMS THAT YOU HAVE READ, UNDERSTAND, AND AGREE TO BE BOUND BY THESE TERMS AND CONDITIONS.
A PASSWORD MUST BE PROVIDED OR REMOVED TO PERFORM A PRE/POST CHECKLIST.
*IF UNABLE TO PERFORM A PRE-CHECKLIST WE DO NOT ASSUME RESPONSIBILITY FOR ADDITIONAL FUNCTIONS NOT WORKING PROPERLY AFTER THE REPAIR. ANY FUNCTIONS NOT WORKING PROPERLY AFTER THE REPAIR ON DEVICES THAT WE CANNOT PERFORM A PRE-CHECKLIST ON ARE NOT COVERED UNDER THE REPAIR WARRANTY AND WILL ONLY BE FIXED AT THE EXPENSE OF THE CUSTOMER.
